Our Experience
At QCM, we provide comprehensive care to improve the health and quality of life to our patients. While providing personal care we ensure each patient is treated with respect and dignity.
Our services are designed to give each patient special attention to decrease the risk of an unwanted ER visit, and necessary hospitalization. This means we do not watch the clock. Each visit is dedicated to the individual, while a typical visit can range from 5 to 15 minutes. We provide at least 30 to 45 minutes for each visit. This is a welcome relief for patients used to rushed office visits that don’t allow enough time to cover every issue in detail.
If requested or needed, we discuss the visit and plan of care with a chosen family member or spouse. We also provide 24/7 mobile contact with ALF for any questions, concerns, or problems that may arise at any given time.

Does QCM accept Medicare?
Yes, we accept primary Medicare. Any other insurance plans please contact them to see if you have out of network benefits.
Does my insurance cover home visits and ALF?
Yes, primary medicare covers 80% and secondary covers 20%. Must contact your secondary to ensure it covers 20% (co-insurance).
Does QCM provide services to nursing homes, hospitals, or rehab facilities?
No! We only are in homes & ALFs.
Does QCM see patients in the office?
No! We only see patients in their homes, and ALFs.
Is QCM accepting new patients?
Yes! Call for an appointment, or complete forms on website.
Can I access my records?
Yes! Through our EMR portal you may access your medical chart summary, message provider, and staff.
